DOH Scholarship

Requirements:

1. TOR

2. Xerox copy of updated PRC ID License

3. Psychometric exam(Mary Chiles College -Guidance Counselor)

4. Medical Certificate

5. NBI Clearance

6. Certificate of Midwifery Experience

7. Certificate of Good Moral conduct from IMAP Chapter or PLGMI

8. Endorsement from Regional DOH Office

 

Other Info:

1. Must be 35 years old and below.

2. Stay In at the DOH assigned dorm.

3. Full scholarship including food, dorm, uniform and shoes.

4. Willing to be deployed.

5. Screening of scholars will be done.
