1. High School Card ( F-138A)
2. For Transferees : H.D. & Scholastic Record
3. Certificate of Good Moral Character
4. Birth Certificate- NSO ( photocopy)
5. 2 long brown envelopes
6. 2 long folders
7. 2 pcs. 2×2 & 2 pcs. 1×1 ID picture
8. Entrance Fee- P200.00

Schedule of Entrance Exam & Interview:
Monday- Friday — 8:00 a.m.- 3:00 p.m
Saturday — 8:00- 11:30 a.m.
CAREGIVER COURSE
Requirements for Enrollment:

1. Resume
2. Birth Certificate ( NSO photocopy)
3. 2 pcs. 2×2 picture
4. 2 pcs. 1×1 picture
5. 1 long brown envelope & folder
Additional Requirements:

1. For High School Graduate- Form 137 A/ High School Card
2. For College Level / Graduate: TOR – Certified true copy
